Klinikum Oldenburg is one of Northwestern Germany’s leading university hospitals, providing highly specialised care across a wide range of medical disciplines. The hospital has approximately 830 beds, while the pharmacy supports a total of around 1,800 beds across connected healthcare units. With a team of around 70 professionals – including eight ward pharmacists – the pharmacy plays a central role in ensuring safe and efficient medication management and serves as a key resource for questions related to rational pharmacotherapy.
As part of daily clinical work, ward pharmacists contribute significantly to medication safety through pharmaceutical validation, participation in ward rounds, and support within the Closed Loop Medication Management process (a patient-specific medication management workflow supporting safe medication use throughout the care pathway). Working closely with physicians and nursing staff, they identify medication-related issues and help optimise drug therapy for patients.

Supporting complex medication therapies

To support everyday clinical pharmacy work and medication reviews, the team uses Medbase Portal. The browser-based platform provides easy access to evidence-based medication information in German, helping assess polypharmacy and demanding treatment combinations.
Using the multisearch functionality, multiple active substances can be reviewed simultaneously to evaluate clinically relevant interactions and pharmacological considerations.
The structured and colour-coded presentation of information helps to quickly identify medication-related risks, while direct access to scientific summaries. The intuitive presentation of information also facilitates the assessment of treatment options and the comparison of potential alternatives.
