At Hannover Medical School (Medizinische Hochschule Hannover, MHH), advanced patient care, research and clinical expertise come together. In this demanding environment, the Central Pharmacy plays an important role in supporting safe and effective medication therapy – particularly when patients have complex medication regimens that change throughout their hospital stay.

Hannover Medical School is an independent university hospital providing highly specialised care to patients from across the region and beyond. It is also one of Germany’s largest transplantation centres, with decades of experience and expertise in transplantation medicine.

Research is an integral part of MHH. Its key research areas include infection and immunity, transplantation and regeneration, and biomedical engineering and implants. By closely connecting research with patient care, MHH works to advance new approaches in diagnostics and treatment.

This expertise is also reflected in the work of the MHH Central Pharmacy. Its 76 employees, including 25 pharmacists, contribute to the quality and safety of medication therapy through pharmaceutical logistics, in-house production and an expanding range of clinical pharmacy services. Pharmacists work closely with physicians and nursing staff to support safe and effective treatment for patients.

Finding what matters in complex medication regimens

The browser-based platform brings relevant medication safety information together in a structured view, helping pharmacists quickly identify important medication-related problems. Results are presented in tables for an at-a-glance overview, while interaction information is supported by literature references that help pharmacists assess its clinical relevance.

Supporting clinical pharmacy in daily practice

At MHH, Medbase Portal is used particularly to support clinical pharmacy work during ward rounds and in medication reviews.

The Central Pharmacy team uses the platform to identify and assess potential medication-related problems, including potential adverse drug effects and risks related to QT prolongation, bleeding and anticholinergic burden. Integrated tables for renal and hepatic function also help pharmacists identify potential dosing issues at an early stage.

Drug–drug interaction checking is another established part of the team’s daily work. Serious interactions are regularly discussed during ward rounds, where pharmacists and the clinical team can consider strategies to avoid or minimise them. Importantly, the information provided by Medbase Portal forms part of a wider professional process. Findings are reviewed by the pharmacy team as part of a professional double-check before being considered in the final treatment recommendation. In this way, the technology supports – rather than replaces – the clinical expertise at the heart of MHH’s approach to medication safety.
